Illinois Youth Survey On May 15th, our school will administer the Illinois Youth Survey to students in grades 6 and 8. The survey is voluntary and we will send a letter home telling you about the survey and there will be a form for you to complete if you desire your child not take the survey. the survey is used to collect data about students' attitudes towards school, drug and alcohol use and other questions regarding family and personal characteristics.
